*Story of refractory difficult to treat schizophrenia.* In 2011 9 year old male, brought by father for angry abusive behaviour, irritability. On Evaluation, patient would think that people are against him, we could hear the voices of people talking about him. Patient was diagnosed as *childhood-onset schizophrenia, * antidopaminergic agents were started. 30 to 40% improvement in 2 to 3 months. In view of less response, combination of drugs started but no response. Even after one year no satisfactory improvement, Clozapine was started, which is supposed to be for refractory schizophrenia. Unfortunately no improvement So brief pulse therapy decision was taken, which were administered by temporally under *general anaesthesia at Chaitanya Hospital Dhule* , under strict supervision of physician and Anesthetist. First time after 2 year patient showed some improvement, 7 courses where done and patient became 100% improved. But after one year he developed, fever, CBC was done, patient had severe *agranulocytosis* , which known side effect of clozapine. So clozapine was stopped. Again patient relapsed with even more severe symptoms, very difficult to handle by parents, severe delusions and hallucinations where present. *Patient tried to commit suicide* . Again option of restarting Clozapine in was given which proper counselling of parents, about side effects. Thankfully this time, patient did not show any adverse effect for *clozapine* . Pt is becoming older, gain insight about illness. Very complaint to monotherapy of Clozapine. Today on 12 Feb he came to *Chaitanya Hospital* for routine checkup. He is feeling 99% better, very occasional delusions and hallucinations. But he is ignoring that now. Most important he is doing his job properly, taluka place he set up his own business, *earning more than 1 lakh per month, * three years back he was in search of job, he's giving employment to other people. He has complete insight about his own illness.
